If you ever use your Backbone as a Headbone, this explains issues with that:

You adjust the level of support (i.e., fullness or flatness) of your Headbone® or Backbone™ by transferring the buckwheat filler through it.

The ideal amount of filler in either Headbone® or Backbone™ equals about twice as much as you may potentially need, because you use about half the filler for your pillow and the other half as counterweight.

Since you need less volume behind your lumbar region than you need behind your neck, Backbone contains about 1/3 less filler than Headbone.

Therefore, if you use the regular Backbone for head/neck support, you'll probably shake most of the filler into the bottom end, leaving the top mostly emptied and lightweight. Since the top counterweight helps keep the pillow in place, your Backbone could slide down and not stay in place as well if you use it this way.

Conversely, you can request more filler in your Backbone (the same amount used in Headbone), but if you use a plumped Backbone for lumbar support, you may end up with an overstuffed top when you make the bottom flat enough for lumbar comfort. For example, look at the photos at right. In order to make the lower end flat enough for lumbar comfort, you could transfer so much extra filler to the top (overhanging) end that it could overflow into the neck beyond the point of overhang. That would force the leftover filler back down the front.

Everyone's needs are unique. So use this info to determine what might work best for you.

The moral of this story may be this: Just get one of each. It's easier! :-)
